Evaluation Study of the Programme for Creativity and Talent Development Winter 2020
Dear Students,
I am Dr. Serene Chan from the Faculty of Education, the University of Hong Kong. I am are now conducting a study titled “Evaluation Study of the Programme for Creativity and Talent Development Winter 2020.” I would like to invite you to participate.
Purpose of the Study
This study aims to evaluate the learning outcomes from the programme.
Procedures
You are invited to complete a questionnaire at the end of the programme. It will last for approximately 15 minutes. A helper, not me, is responsible for collecting the information.
Potential Risks / Discomforts and Their Minimization
The participation of this research study is purely on a voluntary basis. You can choose to stop at any time without negative consequences.
Potential Benefits
The study can help to increase the effectiveness of any future programmes.
Confidentiality
All data obtained will only be used for this evaluation study. Strict confidentiality will be maintained. No personal identifiers will be kept or recorded in the questionnaires.
The data will be kept in the locked cabinet of the office of the researcher at the University of Hong Kong, and in the researcher’s computer with password protection. The data will be destroyed within three years after the data collection of the study.

各位同學：
我是香港大學教育學院的陳穗寧博士，正進行一項<發展創意、施展才華>冬季課程2020的評估研究，我誠邀你參與。
研究目的
這項研究的目的是了解學生參與該課程後的學習成果。
程序
你只需在課程完結前，完成一份有關該課程的評估問卷(需時約十五分鐘)，本人會安排同事收集你所提供的資料。
存在的風險/不安及其減低方法
你的參與純屬自願性質，你可隨時終止參與是項活動，有關決定將不會引致任何不良後果。
存在的得益
有關研究資料將會增加未來同類課程的效能。
保密
所有獲取的資料只供本研究之用，資料絕對保密。收集的評估問卷不會包含個人識別資料。收集的資料會被妥善收藏於研究人員香港大學辦公室內的上鎖文件櫃內，以及在受加密保護的電腦中，所有資料會於收集後的三年內被銷毀。
