Evaluation Study of the Summer Programme for Creativity and Talent Development 2020

Dear Students,

I am Dr. Serene Chan from the Faculty of Education, the University of Hong Kong. I am now conducting a study titled “Evaluation Study of the Summer Programme for Creativity and Talent Development 2020.” The purpose of this study is to evaluate the learning outcomes from the programme. I would like to invite you to participate.

If you agree to join this study, you will be invited to:

Complete a questionnaire at the end of the programme (about 15 minutes).

Your participation is voluntary and you can choose to stop at any time without negative consequences. A helper is responsible for collecting the information. All information obtained will be for research purposes only and kept strictly confidential. No personal identifiers will be kept or recorded in the questionnaires. All of the obtained information will be securely stored in a locked cabinet at the researcher’s office, and in the researcher’s computer with password protection. The data will be destroyed within three years after the data collection of the study.

我是香港大學教育學院陳穗寧博士，正進行一項有關<發展創意、施展才華>暑期課程2020的評估研究，這項研究的目的旨在了解學生參與該課程後的學習成果。我誠邀你參與。

若你同意參與是項研究，你將會被邀請進行以下活動:

在工作坊完結前填寫問卷(約十五分鐘)

你的參與純屬自願性質，你可隨時終止參與是項活動，有關決定將不會引致任何不良後果。亦會有同事替我收集你所提供的資料，所有收集的資料只作研究用途，個人資料將絕對保密。收集的評估問卷不會包含個人識別資料。收集的資料會被妥善收藏於研究人員辦公室內的上鎖文件櫃，以及在受加密保護的電腦中，所有資料會於收集後的三年內被銷毀。
