Evaluation Study of Life Skills Development Online Programme
Dear Students,
I am Dr. Mantak Yuen from the Faculty of Education, the University of Hong Kong. I am are now conducting a study titled “Evaluation Study of Life Skills Development Online Programme”. I would like to invite you to participate.
Purpose of the Study
This study aims to evaluate the learning outcomes from the programme.
Procedures
You are invited to complete an online questionnaire at the end of the programme. It will last for approximately 15 minutes. The programme will be audio-/video-recorded.
Potential Risks / Discomforts and Their Minimization
The participation of this research study is purely on a voluntary basis. You can choose to stop at any time without negative consequences.
Potential Benefits
The study can help to increase the effectiveness of any future programmes.
Confidentiality
All data obtained will only be used for this evaluation study. Strict confidentiality will be maintained. If you do not agree to be video-recorded, you will sit outside the camera view or have your face blurred. No personal identifiers will be kept or recorded in the questionnaires. You will have the right to review the recordings and erase part of or entire recording if you wish.
The data will be stored in the researcher’s computer with password protection and in a locked cabinet in the researcher’s office at the University of Hong Kong. The data, including the data with personal identifiers and the anonymized data, will be destroyed within three years after the data collection of the study.

各位同學：
我是香港大學教育學院的袁文得博士，正進行一項生活技能網上課程評估研究，我誠邀你參與。
研究目的
這項研究的目的是了解學生參與該課程後的學習成果。
程序
你只需在課程完結前，完成一份有關該課程的網上評估問卷(需時約十五分鐘)。
存在的風險/不安及其減低方法
你的參與純屬自願性質，你可隨時終止參與是項活動，有關決定將不會引致任何不良後果。
存在的得益
有關研究資料將會增加未來同類課程的效能。
保密
所有獲取的資料只供本研究之用，資料絕對保密。收集的評估問卷不會包含個人識別資料。如你不同意被錄影，你的影像會在鏡頭以外或錄像會被模糊。收集的資料會被妥善收藏於研究人員香港大學辦公室內的上鎖文件櫃內以及儲存在受加密保護的電腦中，所有含有個人識別或匿名的資料會於收集後的三年內被銷毀。
