Evaluation Study of Life Skills Development Online Programme
Dear Parent/Guardian,
I am Dr. Mantak YUEN from the Faculty of Education at The University of Hong Kong. I am going to conduct a study titled “Evaluation study of the Life Skills Development Online Programme.” Your child is one of the participants in the programme and therefore, cordially invited to take part in the evaluation study of the programme. The study aims to evaluate the learning outcomes of the programme, and it can help to increase the effectiveness of any future programmes.
Students who participate in this study will complete a questionnaire at the end of the programme on the online platform. It will last for approximately 15 minutes. All data obtained will be used for this study only. Strict confidentiality will be maintained. No personal identifiers will be kept or recorded in the questionnaires. The programme will be audio-/video-recorded. Video recording is a group recording. If you do not agree to be video-recorded, your child will sit outside the camera view or have his/her face blurred. You will have the right to review the recordings and erase part of or entire recording if you wish. The data will be stored in the researcher’s computer encrypted with password protection and in a locked cabinet in the researcher’s office at the University of Hong Kong. The data, including the data with personal identifiers and the anonymized data, will be destroyed within three years after the data collection of the study. Participation is entirely voluntary, and your child can choose not to participate in the study. He/She can withdraw from the study at any time without negative consequences.

生活技能網上課程評估研究
敬啓者：
本人是香港大學育學院袁文得博士，正進行這次生活技能網上課程評估研究。由於 貴子弟是該課程的其中一名參加者，故誠邀他參與這項研究。研究旨在了解學生參與該課程後的學習成果，以推動未來同類課程的效能。
參與此研究的同學只需在課程完結前，完成一份有關該課程的網上評估問卷（需時約十五分鐘），所有獲取的資料只供本研究之用，資料絕對保密。收集的評估問卷不會包含個人識別資料。課程將會被錄音/錄影，如您不同意 貴子弟被錄影，貴子弟的影像會在鏡頭以外或錄像會被模糊。您有權檢視錄影/錄音，並有權要求刪除部分或整段錄影/錄音。收集的資料會被妥善收藏於研究人員香港大學辦公室內的上鎖文件櫃內以及儲存在受加密保護的電腦中，所有含有個人識別或匿名的資料會於收集後的三年內被銷毀。是次參與純屬自願性質，貴子弟可不參與此研究或隨時要求退出參與，有關決定將不會引致任何不良後果。
